Offer Details
Book a new patient check-up and clean at Serene Smiles, Perth for $249 (GST-inclusive) (Offer). The Offer includes:
- a comprehensive oral examination (check-up);
- a scale and clean; and
- all x-rays that your dentist considers clinically necessary for your check-up.
Offer Period
This Offer is ongoing and available until withdrawn or amended by Serene Smiles. Serene Smiles may withdraw or amend this Offer, or these terms, at any time without notice.
Redemption Process
To claim this Offer, submit your details via our online enquiry form or contact Serene Smiles directly. Submitting your details does not guarantee you a booking. Serene Smiles will contact you to arrange a suitable appointment time, subject to availability.
You must mention this Offer when booking or at reception before your appointment to redeem it. Payment of the $249 fee is due on the day of your appointment.
Offer Restrictions
The following restrictions apply to the eligibility and use of the offer:
- the Offer is only available to new patients
- this Offer is only available at Serene Smiles;
- must be 18 years or older;
- the Offer cannot be used in conjunction with any other offer, discount or promotion;
- this Offer is subject to professional assessment and clinical suitability determination by our dentist.
*Terms & Conditions
You will be required to complete a medical history form when you attend Serene Smiles and before your appointment, to assist our dentist’s evaluation.
The Offer includes only the services listed under “Offer Details”. Your dentist will determine which x-rays are clinically necessary for you, in accordance with applicable professional standards and radiation safety requirements. The Offer includes all x-rays your dentist considers necessary for your check-up and clean.
To ensure you receive the most appropriate treatment, your dentist will evaluate your teeth and gums at your appointment to consider whether a clean is suitable for you at that appointment. If your dentist considers it is not (for example, you need alternative or additional treatment, such as treatment for gum disease or tooth decay, or a longer appointment), we will provide details of those treatments, including our standard rates. We will otherwise provide the clean at a later agreed date, having regard to your teeth condition. You are not obliged to proceed with any additional treatment.
Any additional or alternative treatment (including treatment recommended following your check-up) is not included in the Offer and will be charged at Serene Smiles’ standard rates. Your dentist will provide a treatment plan and quote for that treatment before it proceeds.
If you are unable to attend your appointment, please give us at least 24 hours’ notice. We may charge a cancellation fee where you cancel or reschedule with less notice or fail to attend.
Health fund rebates may be available for some services, depending on your policy and level of cover. Please check with your health fund. Serene Smiles does not guarantee any rebate amount.
